If assessmente is the act of gathering information and evaluation is the interpretation of that information in order to make decisions for better learning, what kind of information about your ss would you gather and how would you use it to achieve your goals as an English teacher?
Assessments Vs. Evaluation.

Motivation in class.- I need to know if they are motivated, taking part on class, willing to answer (even if they have mistakes). A good attitude!
Speaking.- It is no necessary to correct them every time they talk if they are trying to communicate (speaking) something in English, sing a song, or involving functions as asking, inviting, etc.
Listening.- If they pay attention of what we say on class, if they understand some words of a song, (or are willing to know what the letter of the song means), if they understand when I may ask them something.
Reading.- Monitoring all the readings that Ss can make, according to their age, preferences, and abilities.
Writing.- By doing some sentences for homework, complete some sentences, etc,
Cognitive Skills.- Coloring, matching, ordering, classifying, etc.

how do you gather the information from your ss
I GATHER THE INFORMATION FROM THE SS BY MONITORING THEM IN EVERY ACTIVITY THEY DO, TAKING INTO CONSIDERATION HOW MANY TIMES THEY PARTICIPATE AND HOW WELL THEY DID,
IF THEY APPLY THE THINGS WE ARE WORKING ON.
BY TESTING THEM IN ORAL OR WRITTEN EXAMS, SOMETIMES.
I NOTICE IF THEY HAVE ANY PROGRESS HELPING ME WITH THE FEEDBACK AND THE RECYCLING.
I ALSO REALIZE IF THEY HAVE A PROBLEM WHEN THEY CAN'T ANSWER AN ACTIVITY AS WELL I EXPECTED. THEN I HAVE TO MODIFY THE STRATEGY I USED IT JUST FOR THE PURPOSE I NEED AND USING MORE INTELLIGENCES IN ORDET TO GET MY OBJECTIVE.
SOMETIMES I USED RUBRICS, FOR EXAMPLE IN READINGS, WHEN I NEED TO CEHCK IF MY SS UNDERSTOOD IT.
CHECKLISTS ARE VERY USEFUL 'CAUSE I CAN GATHER THE INFORMATION IN A GENERAL WAY.
RUBRICS I THINK ARE MORE PERSONALIZED.
I ALSO INCLUDE ROLE PLAYS, PROJECTS, PORTFOLIOS.

My Opinion.

It is true that on having evaluated our students it is also a way of evaluating to us, with this form we realize the points we must improve, or in some cases with students of different capacities,it is necessary to know how to support them for a better performance in the Englishman's matter.
The information that I might use is relating to his advance in the comprehension of the language, taking in consideration four aspects: writing, listening, speaking and reading.
With this information I can also notice if it is useful or not for them and also analyse if the way of teaching is enough with all the activities that I bring for them or if I need to improve something or not.

WE MAY ASSESS AND EVALUATE TO SEE HOW WELL THEY HAVE LEARNT THE CONTENTS OF THE WHOLE COURSE.LEARNERS USUALLY RECEIVE A SCORE O MARK FROM THIS KIND OF TESTING AND SOMETIMES FEEDBACK ON THEIR PERFORMANCE.
THERE ARE MANY DIFFERENT ASSESSMENT AND EVALUATION E.G. GAP-FILL,MULTIPLE-CHOICE QUESTIONS,TRUE/FALSE QUESTIONS,ORDERING,CORRECTING MISTAKES,TAKING PART IN INTERVIEWS,CONVERSATIONS FOR ROLE PLAYS,WRITING LEETERS.
THE REASON FOR USING ASSESSMENT AND EVALUATION IS TO HELP LEARNERS TO ONDERSTAND THEIR LANGUAGE USE AND PERFORMANCE BETTER,AND SO BECOME MORE AUTONOMOUS.

Well, I think an assessment is very important due to allow me to have a referece of my students
In order to make a diagnostic and gather specific information in individual such a student performance.
Assessment is also an educational measurment.
I would use for gather information: Daily obervation, test and quizzes, rubrics, rating scales, project work, and portfolios between other things.
Teachers carry out an assessment in order to make a reflective actions for better learning and teaching and make feedback exercises for correcting student. Evaluation is for gather ranks individuals, administrative measure, quantitative. Both serves to make decisions for to have strategies and get the achivments.

Assessment and Evaluation
I gather information of my students monitoring the class, the way they work, if they had some doubt etc and I use to make all kind of quizes, oral, written and all kind of exercises to in order to realize me if they are understanding what I expect, I use to give my students a check list in order they evaluate to me, this is important for me because I know if they are motivate in the class if they like the way I am teaching, etc, this is a way to gather information too. Ask them what do you like to improve or add in our class, with this information is important and help me to improve, modificate,change or whatever the students need in order to get a goo learning.
Evaluation its only to reinforce the knowledge and of course I had to add more things maybe portfolio but I consider the most important in use this kind of evaluations we have to be clear in what do we want to assessment or evaluate. Marisela

The assessment can be carried out at many points during a course, in the classroom or virtual environment, to compare the perceptions of the teacher with those of the students, and to identify gaps between what has been taught and what students have learned. The purpose is to find out what changes they might make in teaching methods or style, course organization in order to improve student learning. And the evaluation is usually conducted at the end of a course. So, maybe just at the end the teachers will know if they have to change, improve a specific point or/and if they achieve their goals.
